Though I'm kinda surprised that Mistake -1 (Half-Life) & Hollow Moon (Unreal Tournament 2004) aren't on this list, those mods creeped me out alot then most AAA & Indie Games.

hollow moon doesnt even have enemies. grow some balls.
Worry of Newport is an HP Lovecraft-esque mod for Crysis 1 that is also worth mentioning.
