Horror has always been an area modders have explored in great detail over the years. In fact one of the first "big name" showcased mods, featured in PC Gamer and plenty of other places (back in the day when PC mags ruled the world) was They Hunger - a horror classic. Since They Hunger many horror themed mods have been released, some dark, some zombie based, some eerieely quiet and some edge of your seat "packed full of action".
So if you are prone to wetting your pants at the slightest scare these are probably not the titles for you. If however you crave something spooky this Halloween - sit back, dim the lights and enjoy ModDBs first ever mod countdown - as we take a look at the top 10 spookiest mods of all time as voted by our community.

The whole problem is people didn't vote for which mods were scariest, they voted for which mods they liked to play most. Too bad that wasn't the point of the poll.
For the life of me, I'll never understand how anyone can find a multiplayer game scary. How the heck can you be scared when you have potentially dozens of other people with you all shooting and chatting away? I think a lot of times people also confuse panic and excitement with fear. There are plenty of times in Zombie Panic: Source where you'll be the last survivor left, low on ammo, and have a swarm of zombies chasing you. That usually causes me to panic (in-game with the panic button and in real-life), but it doesn't terrify me or startle me so badly that I think I'm going to have a heart attack the way a good singleplayer experience can.
Of the 7 games on the list I've played (Hidden, They Hunger, AoM DC, ZPS, Ravenholm, Paranoia, ZM) Afraid of Monsters Director's Cut is without a doubt the scariest. The Haunted and Killing Floor look like a lot of fun, but way too much action to be scary. (And again, they're multiplayer) I've heard Dear Esther is very creepy, but to my understanding it's more or less an adventure game and as such you don't have things jumping out and actually threatening your life. I could be wrong, but most of the footage I've seen shows the player doing a lot of walking around and not much else. Personally, I need enemies to threaten me in order to be scared.

I agree completely with you, no multiplayer game, especially on the Source engine can be scary. Dear Esther is pretty much an interactive story and if you actually pay attention to the story you can get some goosebumps but that's it.
I'm sorry but I don't understand how multiplayer games are scary. I've played both Zombie Panic: Source and Zombie Master and they are far from being even remotely scary. Other mods like Afraid of Monsters, They Hunger, or Paranoia can be scary.
Well people get scared by lots of things, I sure dont know how people are freaked out by snaked but a heap of people are.. So it comes down to what people like and what they voted for.

I agree that Ravenholm belongs on the list. Very creepy.
I also agree that ZPS belongs too- it's very creepy at times. The subway especially; I remember some tense rounds as we tried to get the door open in the storeroom.
you forgot about "mistake" which for sure was a little bit inferior to AoM, but for sure its A REAL SCARY MOD....not just a crappy zombie mod to waste your time with (how "they hunger" and "zombie master" and are into the list....thaz something i just dont understand. they are sure both funny and nice to play, but realy not something "spooky" or "scary".....FOR NOT TALKING ABOUT "THE HUNTED" lol...thaz mod looks retarded).

The *Haunted* is an epic mod. Its a good thing you played it before you even sullied its name :P. If you can run UT3.
Left4Dead (as its out now) is really only tense, but not scary. You don't sit yourself at any moment cause you know regardless of your situation you can mash right click and be safe from almost anything (even on expert). It's only the Boss Zombies that are any trouble and thats only on harder difficulties.
Afraid of Zombies DC is defintely the scarest on these 10 zombie games. I actually find it the only scarest. The rest has some suspense but it's not scary. And if you don't have tried Killing Floor, try it, cause it's damn good.
ok. Im a Serious game/movie horror freak. But is it just me? ore are NO ONE of theys mods Scarry? serious? Zps At 1. Duuude? wtf? i find doom 3 more scarry than theys mods (and yes i know doom 3 is a game not a mod)

i don't get it, when i was voting everyone(including me) was either voting for they hunger or afraid of monsters directors cut. So how the hell did zombie panic get number 1? It's not that it's a bad mod, it's just that it's not scary enough to be number 1. (note: zombie panic is however one of the funnest mods ever)

HL1 and HL2 are harshly overrated on Moddb. Goto play more then 1 (or 2) engines to find the real gold for good mods :P. Ida voted more had I known the community was gonna disregard every good game out there. Glad The Haunted and Killing Floor made it in there.

PPL don't seem to understand that a game is as scary as you make it. With dedicated SERIOUS players (Good players not newbs), any game can startle you. ZP:S is deserving of this list as many of the others. Personally no HL1 mod has ever scared me though paranoia and Cold Blood were the best 2 I've played (and possibly Master Sword which also could have been nominated had anyone ventured the spider caverns 8|) Zombie Masters I've never found to be a very good mod. It seems too much like HL:2 Deathmatch style which ruined it for me.
Maybe make these polls per engine so people who care about the games vote instead of bias and mindless mod spamming. (IE Source, UT, Crytek, ID Tech, etc)
The thing is that EVERYONE knows how to mod for source. It's damn easy. Any 15 year old with enough spare time can learn to use the Source engine to make whatever he wants. Engines like Unreal and the like are harder to learn and aren't as well supported.
The reason there are so many HL and HL2 mods is because there ARE that many HL and HL2 mods, simply because it's so easy to mod.
Zombie panic isn't the only game where players can be infected or turn into zombies.
Brainbread allowed players to become infected and turn into zombies. The human survivors would have to duke it out in a landscape filled with NPC zombies while watching their backs for player-controlled zombies.
I enjoyed both ZP and BB for HL1. I just might give ZP:S a try sometime in the future.
I absolutely loved Paranoia. The soundtrack is just great. I do wish that the team would invest in a sequel that would allow the player to spend a bit more time in the world. I felt that Paranoia was sadly too short and that the ending was a bit anti-climatic. I never really play many single player games or mods but I really loved that one.
Ok. big mistakes were made. If your voting for the scariest mod, then AoMDC shoulda made #1, if its the scarriest anything, i would say the nightmare house mappack for hl2, and if its just the overall best funnest zombie game, then brainbread shoulda made it on the list at least
Hidden Source and Killing Floor have given me some good scares. Not bad list but I would've changed the order around a little ;)

Wow, Paranoia looks amazing! Also, I've been interesting to see more of They Hunger. Can't wait to play that! Killing Floor still looks great, and Dear Esther looks like one of the best adventure games to hit PCs in a while. Afraid Of Monsters looks like a gem, but I don't like the idea of rationing my items very much.
Nice list, but Zombie Panic: Source shouldn't have 1st place. It's nice, but not scary.

paranoia was simply awesome, great graphics, great sound, even the game has an ironsight... IN HALF-LIFE just an amazing mod
these games are all good, some of them look apsolutely pants graphics wise however - why not make the same mod for a later game ? ZPS shouldn't be number one, but then again what will take its place, i suppose ZPS is the best "horror" but i think it's lacking some things.
