I posted this in the SourceMod forums offering $10, and for whatever reason, it went over everyone's heads. Maybe I'll have better luck here.
When a character takes damage, a die equal to its default max HP is rolled. If the roll is greater than its current HP (after subtracting the damage that triggered the roll), the character is reduced to 1 HP. If damage reduces the HP to 0 or less, there is no roll and the character simply dies as usual.
So, for example: A character starts with 200 HP and takes 40 damage, bringing its HP down to 160. A d200 is rolled and lands on 110. Since 110 < 160, nothing happens. Now let's say it takes another 40 damage, bringing its HP down to 120. This time, the d200 rolls 145. Since 145 > 120, the character is reduced to 1 HP. Finally, it once again takes 40 damage, bringing it to -39 HP and resulting in a kill.

Edit: I suppose it's worth mentioning that there's more paid work where this came from, so this would ideally be the start of a series of collaborations.
