When Dog Meat from the original Fallout got disintegrated by a Mutant's rocket launcher :(

Everybody knows that has to be Dom in GoW3. They tugged at my heartstrings hard on that one. Amazing job there. Master Chief had a very good "fake death" but even his fake death wasn't as good as Dom's real one IMO.

You know who had a great death? Tidus, Final Fantasy 10. Mainly because well, you play as him for so long you sorta forget, he is dead. Everything he knows is dead, and then he dies for real.

Agreed on the Ghost part. I'm not a CoD fan by a long shot, but I have to say when I played through the game I found those deaths particularly unfair and in a way, sad. I didnt really give too much of a shit about Roach, but seeing Ghost's death from Roach's POV was immersive, and the player feels truly helpless. Which aside from frustration, is the only emotional response the game elicited.

I don't know about the saddest, but Duncan's in Dragon Age Origins was pretty sad, as was Cailan's.

Mordin in ME3, as he was my favorite character and I didn't see it coming. Serves me right for trying to avoid the genocide of a bunch of bloodthirsty battletoads. Awesome dialogue though:
Shephard: "I'm sorry!"
Mordin: "I'm not."
