Do I have to replay the entire Doom 1 Episode 1 to get to the new version of E1M8b?

Not nessacerely but I would recommend playing from E1M1 and onwards, because it is a bit more difficult then the regular episode.
I copied the e1m8b.wad to the zdoom folder which is v2.7.1 and I load The Ultimate Doom (doom.wad) and I start a new game with Episode 1:Knee Deep in the Dead and I played to mission 8 and still the normal boss level and not the changed one.Am I doing something wrong?

Did you load the map with the -file e1m8b.wad parameter ??
How do I do that? Do I create a shortcut to zdoom with that as a commandline?

Yes, you can do that and the game will load the WAD file.
Additionally, you can add "-warp 1 8" to it *** well so that the game will start automaticly with map 8 (but you'll start with a pistol only).
"C:\My Documents\My Games\zdoom-2.7.1\zdoom.exe" -file em1m8.wad -warp 1 8
This the shortcut that I am trying but it just loads the normal e1m8 map and not the custom map. :( Am I doing something wrong?

Yes, it should be e1m1.wad instead of em1m8.wad. You are referring the game to a *.wad file that does not exist.
Just alter the entire line to the following and you'll be able to play:
C:\My Documents\My Games\zdoom-2.7.1\zdoom.exe" -file e1m8.wad -warp 1 8 -iwad doom.wad
"C:\My Documents\My Games\zdoom-2.7.1\zdoom.exe" -file e1m8b.wad -warp 1 8 -iwad doom.wad
Seems I left out the b in e1m8b.wad and it loads finally! Thanks for the help. :)
If I leave out the -iwad doom.wad from the shortcut will I be able to load the custom map instead of the default e1m8 map when I play the episode from the beginning?

Yeah they are, but that's what makes them fun and re-playable, also you should check out his e1m4b map he made a couple of months later, which then makes doom 1 episode 1 fully his episode, American Mcgee made e1m8 tom hall had done most of e1m4's layout. Anyway check them both out, e1m4b is also a concept of how the envoriment can change quickly which is what Blackroom (John Romero's & Adrian Carmacks FPS) is going to be about.

this only works on the ultimate doom
Is there a way to load this file so when I reach e1m8 normally in game it loads this wad instead of the normal one
The original Doom is somethings I always find myself coming back to although these days with mods attached. This map is a great remake of the original and have added this to my Wadsmooshed collection.
